Update HIVdb to 8.8

There's a new set of rules from the Stanford HIVdb. Look at the changes, and upgrade the rules file.

There have been more rule updates from the Stanford HIVdb project. We are currently using version 8.3, and the following updates are available:

• 8.4 - scoring changes to PI drug atazanavir/r
• 8.5 - new drug: Bictegravir
• 8.6 - scoring changes to INSTI drugs bictegravir (BIC), dolutegravir (DTG), elvitegravir (EVG), and raltegravir (RAL)
• 8.6.1 - no scoring changes, just comment changes
• 8.7 - new NNRTI drug doravirine, and score changes for INSTI drugs bictegravir and dolutegravir
• 8.8 - rule changes for several INSTI drugs, especially DTG

@cbrumme approved the update to version 8.8, with the usual validation process of the next MiCall release. Also, a reminder to update the HIVdb version number on the report, plus a request for the ordering on the report:

We should also modify the PRRT reports to display Doravirine results in the NNRTI section, and the INT reports to display bictegravir results. Both of these should be displayed as the last entry in their respective sections (As, my recollection is that we are displaying drugs in the order that they were approved. Not in alphabetical order. If my recollection is wrong, then please continue with the current ordering scheme).
