We need to create a Google Form Template for HfLA Interest form so that our external outreach has a consistent HfLA look and feel.

We need to create a series of templates to suits different communication needs for the Civic Opportunity project:

• Template for interest form
• Template for pipeline intake form
  • Create the header image - size 1600 * 400 px

The end goal is to produce a style guide for document creation

I added the list of projects with their one-line descriptions to the template. Projects can be added or deleted from the list based on the type of interest form.

@tan-zhou - What do you think about us duplicating the BOP Template that Bonnie shared with us the other day? I've copied it into our Project Drive just in case we want to mimic this guide and create links that would live in one single location.
