Test: NPM Scripts Explorer enhancements

Several enhancements were done to the NPM Scripts Explorer:

• pre/post script are no longer filtered but rendered with a different icon
• inline actions for run and debug (when the script is a debug script)
• you can run npm install from a package node in the tree
• new setting npm.scriptExplorerAction to control the default click action
• the view is automatically refreshed when the npm.exclude setting is changed
