This patch makes several changes to the default templates for
new moderation actions:
- Remove currently-unnecessary "kickban abuse", "leaving early",
and "position leave/stop" templates.
- New templates for "passing info", "unpausing", and "politics".
- New "addon" templates for frequent/intense offenses and bot
games.
- Update CoC rule reference (A4->A3) based on the most recent CoC.
- Include the original ("bad") username in the "rename" template.
- Make punctuation/capitalization consistent for all templates.
