Added support for installing the manhole via the PYTHONMANHOLE
environment variable.
Added a strict install option. Set it to false to avoid getting
the AlreadyInstalled exception.
Added a manhole-cli script that emulates
socat readline unix-connect:/tmp/manhole-1234.
