Consistently format N=x with plain equal sign.

Do not put equal sign inside \fI as it ends up big and ugly.  Define
string \*(eq to consistently supply \^ around the equal sign and use
it everywhere where the equal sign is used without explicit spaces
around it.  Ditto for \(!=.  Makes it easier to grep too.
