fix for wp 4.2

- deprecate shell_tinymce_2(), "hr" already available in wp 3.9
- remove "underline" mce button in shell_tinymce_1(), already available in wp 3.9
- in shell-open-sans, use all font variation, and remove "http:" for compat with "https" protocol.
- add/register shell-merriweather google font.
  now we have serif (Merriweather) and sans-serif (Open Sans) google fonts to use :)
- deprecate shell_get_menu_location() and add shell_get_menu_name( $location )
  for a better function name.

- update respond.js to version 1.4.2

- add "shell_get_menu_location()" to use in menu navigation toggle.

- add "shell_entry_meta()" that will return shortcode for all taxonomy/term attach to a post/entry.
- update translation file, now "navigation" is not needed.
